Accident/Critical Illness Wellness Benefit
Did you know about the Wellness Benefit with Voya? If you enrolled in the Accident or Critical Illness plans with Voya, you can receive $50 for yourself, $50 for your spouse, and $50 for each child on your plan. If enrolled in both ACC/CI plans, you can receive $100 for each covered individual ($50 for Accident Insurance and $50 for Critical Illness).
What is the Accident/Critical Illness Wellness Benefit?
The Wellness Benefit is included with your Accident and Critical Illness Insurance coverage. It provides an annual benefit payment if you complete a health screening test on or after your coverage effective date, whether there is any out-of-pocket cost to you.
- You only need to complete one health screening test and may only receive a benefit payment once per calendar year, even if you complete multiple tests.
- You may also receive an additional $50 benefit payment for your spouse and/or children if they are covered on under your ACC/CI plan(s).
- If enrolled in both ACC/CI plans, you may submit the same screening for both ACC/CI and can earn $100 for each covered individual for whom an eligible wellness benefit claim was filed ($50 for Accident Insurance and $50 for Critical Illness).
How To Get Your Wellness Benefit
- You and your covered spouse and/or children complete a health screening test (you can use the same screenings/exams you submitted to Wellworks). You are not required to submit documentation to Voya.
2. Visit your Voya Employee Benefits Resource Center at https://presents.voya.com/EBRC/PLMarketing

- Click on “File a Claim”
- Scroll down to the bottom of the page to where it says “Remember to file your annual Wellness Benefit Claim” and click “Get Started”
- Complete the questions regarding the health screening test, electronically sign and submit your Wellness Benefit claim. You are not required to submit documentation to Voya from the screening/exam.
- A confirmation number will be provided for your reference, as well as the option to save the form for your records.
- Receive a benefit payment for each covered individual for whom an eligible Wellness Benefit claim was filed.
