Find a Dentist
For the best savings, use a Sunlife participating dentist or specialist. You can find a dentist by visiting www.sunlife.com/findadentist. If you choose a dentist who does not participate in our dental plan, your out-of-pocket expenses may be more, since you will be responsible for paying any difference between the dentist’s fee and the plan’s payment for the approved service.

- Go to Vision Provider Locator (eyemedvisioncare.com)
- Choose your vision network from the drop down. (PLM is in the Insight Network)
- Enter either your 5-digit zip code or select “Use my location.”
